CODE OF CONDUCT
AGREEMENT AND RELEASE FORM

  • I recognize that the NBPCB maintains certification of SD Professionals and adjustment centers in such a way so as to make its certification a desired and valuable credential, that the NBPCB publishes the names and certification statis of SD professionals and adjustment centers on the NBPCB web page, and that NBPCB provides me and my staff with valuable ongoing continued education training to help maintain knowledge, skills, and certification.
  • I hereby acknowledge that I have read, understand, and agree to abide by the Code of Conduct and Grievance Procedures as established by the National Blindness Professional Certification Board as a condition of my, or the adjustment center's of which I direct, Structured Discovery Professional Certification.
  • I affirm to conduct myself to both the letter and the spirit of the Code of Conduct, including conforming to all aspects of the tenets of Structured Discovery methodology, beneficence, nonmaleficence, privacy and confidentiality, professional duty, justice, veracity, and fidelity. I understand that if I am found to be in violation of the NBPCB's Code of Conduct my certification, or that of the adjustment center of which I direct, and all privileges therein may be permanently revoked.
  • I understand that instances of failure to comply with the entirety of the Code of Conduct and any forms of abuse or neglect may substantially and materially impact on the provision of Structured Discovery instruction, and may be grounds for permanent revocation of certification.
  • I further understand that a condition of obtaining and retaining NBPCB certification requires SD professionals and certified centers to follow all applicable state laws with respect to the reporting of abuse and neglect, to maintain knowledge of applicable abuse and neglect laws, and to provide information to consumers and staff on the reporting of allegations of abuse and neglect.
  • I recognize that in some circumstances my obligation to report abuse or neglect to law enforcement or protective services may delay reporting to NBPCB, but that while this delay may be necessary to avoid interference with their investigation, I am still obligated to report to NBPCB once I am released to do so my law enforcement authorities.
  • I affirm that I have reported to NBPCB of any investigations in which I am a subject involving misconduct or abuse that has resulted in my resignation, termination, suspension, or discipline.
  • I further affirm that I have never been charged with or convicted of a crime that involves any abuse of a child or vulnerable adult.
  • I hereby grant permission for any SD Professional to release any and all information regarding my conduct or behavior which may be relevant to any investigation if I am accused of being in violation of the NBPCB's Code of Conduct, including but not limited to information about disciplinary action, reprimands, matters of opinion, information about my character, and information about my conduct. I further agree to release and hold harmless any SD Professional from any and all claims, causes of action, and liability that may result from releasing such information, including but not limited to claims of defamation, slander, libel, negligence, or interference with contract.
  • I hereby grant permission for my current and previous employers to release any and all information regarding my conduct or behavior which may be relevant to any investigation if I am accused of being in violation of the NBPCB's Code of Conduct, including but not limited to information about disciplinary action, reprimands, suspensions, matters of opinion, information about my character, and information about my conduct. I further agree to release and hold harmless my current and former employers from any and all claims, causes of action, and liability that may result from releasing such information, including but not limited to claims of defamation, slander, libel, negligence, breach of contract, or interference with contract.
  • “I hereby grant permission for any school, college, or institution of higher education I am or previously attended; any vocational rehabilitation agencies from which I currently or previously received services; any group, program, volunteer organization, or membership organization that I previously belong to or was affiliated with, to release any and all information regarding my conduct or behavior which may be relevant to any investigation if I am accused of being in violation of the NBPCB's Code of Conduct, including but not limited to information about disciplinary action, reprimands, suspensions, matters of opinion, information about my character, and information about my conduct. I further agree to release and hold harmless these entities from any and all claims, causes of action, and liability that may result from releasing such information, including but not limited to claims of defamation, slander, libel, negligence, breach of contract, or interference with contract.
  • I hereby grant permission for the NBPCB to inform my employer, other certification or license boards, schools, colleges, or institutions of higher education, vocational rehabilitation agencies, groups, programs, volunteer organizations, or membership organizations, protective agencies, or law enforcement, of my certification status, including certification revocation, and to release any and all information regarding the results of an investigation, including preliminary conclusions or incomplete investigations, pertaining to my conduct or behavior which may be relevant to any violation of the NBPCB's Code of Conduct, including but not limited to information about disciplinary action, reprimands, matters of opinion, information about my character, and information about my conduct. I further agree to release and hold harmless the NBPCB from any and all claims, causes of action, and liability that may result from releasing such information, including but not limited to claims of defamation, slander, libel, negligence, breach of contract, or interference with contract.
  • I acknowledge that NBPCB may conduct a background check as part of my initial certification or as part of my recertification process and I hereby authorize NBPCB to use entities such as the National Sex Offender Public Website and other background checking sources.
  • I acknowledge that the National Blindness Professional Certification Board reserves the right to change these terms and conditions at any time without prior notice and that any changes will be reflected on the NBPCB website.
